Today marks the start of Palliative Care Week, a government initiative aimed at raising awareness of the need to develop an early palliative care plan. Being prepared for end of life care allows patients to maintain dignity and a sense of identity whether at home, in aged care or in hospital. By involving family and GP’s in a palliative care plan ensures open communication and eases the transition.
