An Indigenous man has died in hospital, after an attempt to take his own life in a West Australian prison. He had been threatening self-harm, but hadn’t been placed in an ‘observation cell’. Instead he was put under 6 hourly observation, which a psychologist eventually downgraded, to twelve hourly checks. He was later found hanging in his cell.
