After a $40 million ocean recovery mission and three years of investigation, the final report into the crash of Air France Flight 447 has been released, and it asks some serious questions of the modern aviation industry.

The report, published by French investigators BEA, found the pilots were negligent in their handling of the Airbus A330 jet. They took improper steps to manage a loss of airspeed instrument data, causing the jet to stall at 38,000 feet and plummet into the Atlantic Ocean. All 228 on board were killed instantly. The final report is set to herald changes in training procedures, flight deck displays and industry regulation around the globe.
